The course includes education on the Hospital Readmission Penalty Program, nursing home Readmission Penalty information, Bundled Payments for Care Improvement Program (BPCI) and BPCI Advanced Program. The session reviews the origins of the Patient Protection & Affordable Care Act of 2010, why the various programs were necessary, what the goals of more effective care coordination are and tactics on how to effectively manage care transitions to ensure quality care is delivered, patient safety, and effective cost management.
The session includes an instruction sheet, an interactive video, an accompanying power point presentation and a competency exam at the conclusion. Participants who successfully complete the exam will automatically be provided a completion certificate.
