The Certified Home Care Fellow Team program allows teams to complete a brief online independent study course individually or as a group. Once your team complete this course, you will stand-out from the competition as you will have a much deeper understanding of how to most appropriately address care transitions and issues across the continuum, and position yourself and your organization as a preferred provider in your local community. Complete at your own pace. Don’t miss out on this fantastic program.

Up to five team members will receive the certification.

Upon completion you will receive:

• A fellow certification certificate that can be posted in your office
• A press release you can complete to issue locally
• A Certification Seal that can be included on your business cards, website, email signature and brochures

The not-for-profit National Readmission Prevention Collaborative has been offering Certifications to individuals and teams since 2014. More than 400 individuals and teams nationally have completed the online program and proudly attach the Certification Seal to their business card, email signature, website and brochures. The Fellow and Team programs are identical programs, although one is designed for individuals to complete online, and the other is simply re-formatted to be completed in a group setting.
