The not-for-profit National Readmission Prevention Collaborative has been offering the Certification in Readmission Prevention to individuals and teams since 2014. More than 200 individuals and teams nationally have completed the online certification program and proudly attach the Certified Readmission Fellow Seal to their business card, email signature, website and brochures. The Fellow and Team programs are identical programs, although one is designed for individuals to complete online, and the other is simply re-formatted to be completed in a group setting. Both take approximately 3 hours or less and in 2019 the program was updated and revised to include 4.0 Continuing Education Units (CEU) for Registered Nurses, Case Managers, Social Workers and Nursing Home Administrators, as well as 4.0 Continuing Medical Education (CME) for physicians and PharmD.

This course can be completed at your own pace and is all online. Although the team program allows up to four individuals on your team to share the Team Certification Seal upon completion, up to 3 individuals can also complete up to 4.0 CEU/CME each. Prior certified Fellow’s and teams include Case managers, Social Workers, registered nurses, utilization review professionals, navigators, physicians, pharmacists and anyone working in the transitional care space. Additionally the program is popular with home health and home care teams, placement coordinators, assisted living facilities and nursing home administrators and D.O.N.’s looking to set themselves apart from the competition and get education credits as well. Once certified, those certified may also request permission to promote, provide and offer this CEU course to potential referral source sin the hospital!
